The data collected ensures I have the right information for assessing your suitability for treatment, for contacting you regarding follow ups, exercises plans or any referrals to GP or other healthcare practitioners if deemed necessary. I only collect data that is relevant to those purposes. All information held will be treated as strictly confidential and will only be released with consent of the client.

I acknowledge that the therapist is not a physician and does not diagnose illness or disease or any other physical or mental disorder. I clearly understand that sports therapy is not a substitute for a medical examination. It is recommended that I attend my GP for any ailments that I may be experiencing. I acknowledge that no assurance or guarantee has been provided to me as to the results of the treatment.  

Declared and informed consent 

I confirm that the above information is correct to the best of my knowledge. if there is any change in my condition I will notify the therapist at the earliest opportunity. I understand that there will be no liability on the therapist part should I forget to do so. I understand that this therapy may involve a combination of techniques including a physical assessment, sports massage, remedial soft tissue techniques, remedial exercises and stretches. 

I understand that all treatment, possible contra action and aftercare will be explained to me prior to treatment and I give consent to the treatment provided.
